Pavlik harness

It is common to have questions as you and your child get used to the harness.

This step-by-step video shows the correct way to fit your baby’s Pavlik harness.

Frequently asked questions

We have compiled answers to some of the most common queries below.

What causes developmental dysplasia of the hip (DDH)?
The exact cause is unknown, but DDH is more common in first-borns, girls, breech births, and those with a family history.
Will the Pavlik rub on my baby's skin and what can I do to prevent this?

Some mild skin irritation may occur in some areas due to rubbing, which may result in redness.

If the redness is blanching (fades when you press on it), it’s generally not a cause for concern.

To help prevent rubbing, you can purchase or make harness covers. Alternatively, a simple option is to cut the toes off a pair of socks and slide them onto the straps to act as soft covers.

If you notice persistent redness or broken skin, please contact the orthotics department.

Why can't I put all clothes underneath the brace?

To help maximise the time your baby spends in the Pavlik harness, it’s important that nappy changes can be done without removing the brace.

When dressing your baby:

  • Clothing without legs (e.g. singlets or bodysuits) can be worn under the harness.
  • Clothing with legs (e.g. onesies, leggings, or pants) must be worn over the harness.
What is the best clothing for my baby when wearing the brace?

Avoid tight leggings and pants as they will pull your baby’s knees together.

Upsizing clothing should provide some room over the knees and prevent them being pulled towards each other.

If the clothes you have at home are not working well over the Pavlik, some outlets offer clothing specifically for use with the Pavlik braces, including:

How much does the Pavlik weigh?

The Pavlik harness weighs approximately 100 grams.

How do I clean the brace?

Most cleaning should be done via spot cleaning.

Do not machine wash the brace, as this can damage the Velcro and affect its function.

The Pavlik harness can be hand-washed using mild, soapy water, but it must be completely dry before being put back on your baby.

It’s normal for the brace to become a little dirty over time. Please only wash the brace if it is very necessary. It is vital to maximise the time your baby is in their brace.

Strap covers can be used to help keep the brace clean, reducing contact with skin.

Wearing clothes over the brace can also help to keep the brace clean especially if your baby spits up a lot.

Can I use a baby carrier?

Yes, you may use a baby carrier while your baby is wearing the brace as long as it keeps their legs apart in the 'frog leg' position.

Please do not use a hammock style sling carrier.

Can I use a sleep sack/bag/suit?

Yes.

A sleep sack/bag/suit can be used if it is not too tight and doesn’t pull your baby’s knees in towards each other. Often a size up is required when your baby is wearing the brace.

If what you have at home isn’t working and you’re looking to purchase, there are many brands that sell hip brace specific sleep sacks, including:

Is my car seat too narrow?

It is likely that your baby will fit into your car seat, although as they grow it may become tight across the knees, particularly if using a capsule car seat.

If you are unsure whether your baby is fitting correctly in their car seat, please email a photo of your baby in the car seat to the orthotics department.

We will review the photo and let you know if the positioning looks appropriate.

If needed, we can arrange an appointment with an Occupational Therapist (OT) who can assist with adjusting the car seat setup.

Is it ok if my baby moves its legs when the brace is on?

The Pavlik harness is designed to limit certain movements to support your baby’s hip development, but it does not restrict movement completely.

Your baby will be able to move within the brace. The goal is to guide the hips into a healthy position while allowing natural movement that supports development.

If you have any concerns about how your baby is moving in the harness, or if they have not been kicking their legs as much as you would expect, please contact the orthotics department.

How long will my baby need to wear the brace?

The length of time a baby needs to wear the brace varies depending on the severity of the DDH.

The orthopaedic doctor will decide on the length of treatment based on how the hip development progresses over time.

Can I have another brace?

At PCH we provide one brace per child to ensure we can provide a service to all babies referred within Western Australia.

When your baby grows out of the brace, they will be provided with a new, well-fitting brace.

If you purchase a second brace from elsewhere, PCH orthotics will not adjust this.

What to expect from a video call follow up appointment

In a video call follow up appointment, we will adjust your baby's brace to accommodate growth.

It is best to use your mobile phone or tablet for the appointment as we will ask you to hover the phone over your baby for a bird's eye view of the hips and the brace.

First, we will check the shoulder straps. There should be a gap between your baby’s armpit and the chest strap of the brace.

Next, we will check the position of your baby’s hips and legs. If required, the orthotist will then advise you to lengthen or tighten the front and rear leg straps.

Following the appointment, please use a permanent marker to mark the straps.
