If you need to update a patient's address and contact details or change a patient's name, please use the forms below.

Change patient contact details

• Complete a patient change of details or contact details form (fillable PDF 200KB)
• Email completed form to PCH.pmiofficer@health.wa.gov.au

Change of patient name

• Complete a change of patient name form (fillable PDF 138KB)
• Provide proof of name change documentation (photocopied and attached to email)
• Email completed form to PCH.pmiofficer@health.wa.gov.au

Change a medical record

To change the content of a medical record, you must provide a written request to Release of Information.
