Koorliny Moort

Who we are

Koorliny Moort helps families navigate our health service by:
  • coordinating appointments to minimise the need for families to travel to PCH
  • offering appointments closer to family home
  • ensuring essential information is shared with other health services involved in patient care, including GPs
  • providing health advice and social, cultural and family support
  • helping to arrange Telehealth appointments
  • collaborating with Aboriginal Community Controlled Health Services (ACCHS) and other community services to help families meet their health care needs.

We offer fortnightly clinics at PCH and weekly clinics in the community. Community clinics are delivered in conjunction with Derbarl Yerrigan Health Service, Babbingur Mia Health Service, and with the CAMHS Aboriginal Mental Health and CAHS Community Health Aboriginal Health teams.

The program works with Aboriginal families to provide support anywhere children and young people on their health journeys.

We see patients and clients with complex medical and or psychosocial needs who require services across a range of health and community services. The Koorliny Moort team provides a consistent healthcare professional to facilitate, advocate, and coordinate care for Aboriginal patients and clients.

Services we provide

Rural care coordination

Aboriginal children and young people who are 15 years old or under, who live in Western Australia and have any planned hospital appointments at PCH can be referred to Koorliny Moort by their doctor or other health professionals.

We help improve the coordination of care for children who come to hospital on a regular basis by:

  • coordinating appointments to reduce the need for families to travel to PCH
  • offering appointments closer to home
  • ensuring essential information is shared with other health services involved in patient care, including GPs
  • providing health advice and social, cultural and family support
  • helping to arrange Telehealth appointments
  • collaborating with Aboriginal Community Controlled Health Services (ACCHS) and other community services to help families meet their health care needs.

The Koorliny Moort team can also assist families with discharge arrangements, follow up appointments and outpatient referrals to PCH.

Metropolitan care coordination

Koorliny Moort’s nursing-led care coordination helps to:

  • combine and coordinate appointments and travel
  • offer choices for appointments closer to the family’s home
  • helps find medical records and results
  • improves hospital planning and communication
  • provides health advice, social and cultural family support
  • initiates and arranges Telehealth services.

Paediatric clinics

Koorliny Moort delivers fortnightly clinics at PCH and weekly clinics in the community, all clinics are attended by a pediatrician and a nurse.

A paediatrician is a specialist doctor who improves the health of children and young people. They diagnose a wide range of medical conditions and treat infants, children and young people. They care for children and young people with day-to-day illnesses, minor injuries and infectious diseases. Paediatricians can also specialise in many areas of health related to children including development, behaviour, hormones (endocrinology) or heart conditions (cardiology).

Community paediatric clinics are delivered with Derbarl Yerrigan Health Service (DYHS) at their Mirrabooka, Maddington and Midland clinics, with CAHS Community Health at their clinics in Kwinana, Bentley, Mirrabooka and Hilton, at Babbingur Mia in Rockingham and at the CAMHS sites.

How to refer

Referrals are received via eReferrals within PCH, and via the Central Referral Service via GPs and health professionals outside of PCH. 

All referrals to Koorliny Moort must have a completed Koorliny Moort referral form. Please refer to our Pre-Referral Guidelines.

Eligibility

Care Coordination

  • Aboriginal children and young people who are up to 16 years of age
  • Must live anywhere in Western Australia
  • Have planned hospital appointments at PCH with two or more specialties
  • Have complicated health care needs.
Please discuss with the Koorliny Moort team prior to referral.

Paediatric clinics

  • Aboriginal children and young people up to 16 years of age who reside in the metropolitan area
  • Require assessment by a paediatrician. 

Family to consent to the referral to Koorliny Moort.
