Hospital Liaison GP

PCH’s Hospital Liaison General Practitioner (HLGP) provides a central link for efficient and timely communication between the hospital and GPs and their practice staff.  

The HLGP service is provided solely to GPs and their practice staff and is not intended for use by patients or their carers or families. 

PCH’s HLGP is on site on Tuesday mornings and Wednesday afternoons. 

To speak to the Hospital Liaison GP, please phone 6456 3317If the office is unattended, please leave a message. You may also phone Post Graduate Medical Education Administrative Assistant, Karen Stevens, on 9456 0503 to leave a message.  

Emergency admission to PCH

Please call the switchboard on 6456 2222 and ask to speak to the emergency registrar for all emergency admissions. 

This service is available 24 hours, seven days a week.

Outpatient referrals

Routine non-urgent

Please use the Central Referral Service (external site) for all referrals. 

Urgent (to be seen in less than 7 days)

Please call 6456 2222 and ask for the registrar for the department required. Please ensure you note the name of the PCH doctor who authorised the referral on the referral letter and fax it to 6456 0097.

Health Records Management department

To obtain medical records, please fax PCH Medical Records on 6456 2400 and provide the following details: 

  • patient first name, family name and middle name if possible
  • patient date of birth and, if known, Unit Record Number
  • signed consent of the patient's Next of Kin, unless the patient has been recently discharged to the care of the GP who is requesting the records
