Who we are

PCH's two home visiting services – Hospital in the Home (HiTH) and Post Acute Care (PAC) – enable children to receive treatment and care in their home environment as an alternative to staying in, or returning to, the hospital. 

Services we provide

HiTH provides home-based acute care for medically stable children who require regular health services, and who would otherwise be hospital inpatients.

PAC delivers acute and chronic care for children at their home following discharge from hospital.

Our nurses provide treatments such as intravenous antibiotics, dressings, clinical assessments, central venous access devices (CVAD) care and parental support.

The HiTH and PAC nurses work as one team and provide daily home visits (up to three per day for each HiTH patient) from 7am to 10pm.

Referrals

Your treating health professional can advise whether home visit services could be suitable for your child, based on the best treatment options to suit their individual circumstances.

Our services accept children who have a diagnosis, are medically stable, do not require intensive medical interventions and fulfil a range of criteria regarding parent/carer involvement and availability, geographical location and the safety and suitability of the home environment.

HiTH and PAC nursing offices (based at PCH’s Day Treatment Unit): 7am - 10pm, seven days a week
