Statewide cross-sector care coordination service

The Rare Care Centre works with children and their families to identify their unmet needs and navigate, integrate and coordinate services and care on their behalf.

The cross-sector team members work in collaboration to bridge gaps and link families to services including education, mental health, primary care, genetics counselling and the National Disability Insurance Scheme (NDIS).

Where is the Rare Care Centre located?

The Rare Care team is located at Perth Children’s Hospital.

The Rare Care clinical appointments are held at REACH Clinic, located within North Metropolitan TAFE Mt Lawley.

Referrals

Referrals should be made by a child's primary medical team at Perth Children's Hospital and will remain under the care of their referring team whilst receiving services from the Rare Care Centre. 

Referrals can be made via WA e-referral. Each referral will be assessed by the Rare Care Intake Team.

Please note: Rare Care is an outpatient and time-limited service only. As such we are unable to admit patients and will work with existing medical teams to improve outcomes.

Eligibility Criteria

  • Has a rare or undiagnosed disease (affects less than 1 in 2000 people, including undiagnosed patients with high probability of rare disease)
  • Has an identified current gap in service need (e.g. education, NDIS, genetic counselling, GP)
  • Under the care of a PCH medical team

External referrals are currently not accepted from GPs and private providers to Rare Care.  Please contact the child’s primary PCH team to discuss the potential for referral to Rare Care.
